Primary physical & mental health care of patients/families in their homes. Coordinating care with any other health care providers in the family's system of care. Interface with insurances, billing, & administration duties. Approximately 20 hours/week.

Mental Health Psychopharmacology in conjunction with clients' therapists. Approximately 4 hours/week in rural Madison County.

Private primary mental health care practice of predominantly children and adolescents. Intake assessment, continuing medication management and psychotherapy. Approximately 15 hours/week.

Providing consultation and medication management of children and adolescents who are being served by social work therapists. Approximately 6 hours/week until they downsized.

Admission history and physicals on primarily children and adolescent units. Also cover adult and specialty units. Episodic treatment and coordinated patient medical care with community providers, inpatient psychiatrists, and internist consultants. Initially full time, then on call.

Admission and follow through to a caseload of 30 to 40 clients in home care or 80 to 100 in Personal Care Program.
Coordinate with other community resources including DSS.
Patient/family teaching & advocacy.

Start up of three county certified home health agency within a traditional contract and staff relief business including three office systems of reporting & operations.
Interface with a corporate group not familiar with NYS DOH and DSS regulations.
Identification and training of all professional staff, certificate training PCA's & HHA's.
Coordinate & supervise all delivered services. Identify and negotiate therapist contracts.
Initial patient assessments and screening of potential clients for appropriate programs.
Community relations and marketing. Interpretation of monthly P&L, Payroll weekly.
Set up of statistical and reporting systems. Quality Assurance & Utilization Review.
Input in the national revision of the Kimberly-Quality Care nursing and procedure manuals.

Reorganize and update a functioning LTHHCP; Retrain professional staff.
Restructure financial reporting with statistical reports to improve costs.
Participate in CON process for expansion. Revamp nursing procedure manual.
Upgrade and rewrite documentation procedures, job descriptions, philosophy, and purpose.
Analyze and initiate marketing, including Public Speaking.
Quality Assurance and Utilization Review; Chair Medical and Professional Advisories.

Coordinate, authorize, and document services for insurer, providers, lawyers, & patients.
Evaluate, recommend and document to insurer any job retraining or modifications.
